Interim Waiver & Trial Membership Agreement

This waiver applies to all trial sessions, introductory offers, or temporary training arrangements at Fusion Martial Arts Centre Ltd.
It must be read, understood, and signed before taking part in any class.

DECLARATION

I, {name}, wish to take part in training at Fusion Martial Arts Centre Ltd on a trial or temporary basis.

I understand that participation in combat sports involves physical contact and carries a risk of injury. I agree to participate entirely at my own risk and indemnify Fusion Martial Arts Centre Ltd, its instructors, officers, and members against liability for injury, loss, or damage, however caused, except where required by law.

TERMS & CONDITIONS

• I understand that during the trial period I am covered by basic insurance only, limited to supervised combat sports training.
• Cover does not extend to:
  • Use of free weights or training tools (e.g. hammers) without a prior induction.
  • Sparring inside the cage
• Any such activities are undertaken entirely at my own risk.
• I will report any accidents or injuries immediately to an instructor and inform the Centre if medical attention is later required.

MEDICAL CONDITIONS

• I confirm that, to my knowledge, I have no medical condition that would affect my ability to take part in combat sports.
• If unsure, I will seek advice from a GP or medical professional before participating.

SAFETY & EMERGENCY PROCEDURES

• I have been informed of Fusion Martial Arts Centre’s fire evacuation procedure and emergency exits.
• I will follow staff instructions at all times in the event of an emergency.

CHILD PROTECTION & SAFEGUARDING

• Fusion Martial Arts Centre Ltd prioritises child safety. Any safeguarding concerns will be reported to the relevant authorities.
• Only parents/guardians of participating children may spectate.
• Instructors are DBS checked, and children may be left during the class session but must be collected promptly afterwards.
• The Centre is not responsible for children left unattended outside the premises.

PHOTOGRAPHY & MEDIA

• No photography or filming is permitted without prior consent from the instructor and the parties involved.
• I consent that Fusion Martial Arts Centre Ltd may capture photos or video during sessions for use on the Centre’s website and social media. Content will not be shared with third parties.

COMMUNICATION

• Fusion Martial Arts Centre Ltd uses WhatsApp to communicate with members about class changes, gradings, competitions, and other important updates.
• I understand that it is my responsibility to join the group to receive these updates.

AGREEMENT

By signing below, I confirm that I have read and understood this interim waiver, and that I accept the risks and conditions of participation in combat sports training during trial or temporary membership.
