The following policy and statement should be read and agreed to, prior to signing the Licence and Annual Contract document.
The document is your/your child's consent to a standard membership with the Fusion Martial Arts Centre Ltd.
LICENCE
DECLARATION
I {name} wish to apply for an individual membership and standard insurance with the Fusion Martial Arts Centre Ltd.
Notwithstanding the provisions of the insurance I hereby agree to indemnify jointly and severally the chief instructor, officers and instructors of Fusion Martial Arts Centre Ltd, together with all other members in respect of all liability; however arising, whether by negligence, breach of contract or breach of statutory duty, including the common duty of care imposed by the occupiers liability act of 1957, for the death or personal injury to the aforementioned person or any other person occasioned whether on or off the group premises, by act of default of the member.
NOTICE: Whilst every care will be taken to ensure your safety during training or in competition, you are reminded that the practice of martial arts can involve a risk of personal injury to yourself or others.
TERMS AND CONDITIONS
I accept the terms and conditions of the standard insurance and it is my responsibility to ensure that my cover is current (failure to do so will incur a £5 fine) .
I am aware that cover does not include the use of free-weights, hammers or accidents/injuries obtained whilst doing so nor does it include training or sparring inside the cage. In doing so, it will be done entirely at my own risk. Neither Fusion Martial Arts Centre Ltd, instructors or students will be held liable, should such an incident occur.
I will report immediately, any accidents or injuries sustained during a lesson to an instructor or if any medical attention/hospital visits are sought afterwards.
I/we understand the necessity to be punctual to sessions allowing sufficient time to warm-up and stretch-off and therefore a strict 15min cut-off will be in effect for all session. Arrivals after this time can cause disruption to the organisation and planning of the session and therefore training will not be permitted.
I/we agree to purchase an annual licence at the cost of £15 per annum (per person) which is non-refundable .
I/we also acknowledge that annual renewals may be collected automatically from the payment card I/we provided.
TUITION
My/our tuition will be calculated over a 48 week year and paid in 12 monthly installments, on the 1st of each month; thus allowing a 4-week period of FREE training (examples would include summer/winter holidays or breaks for religious practices such as fasting).
FREEZING POLICY
I/we understand that tuition may only be frozen due to reasons such as an injury or long term illness. A request must first be submitted via email to fusion_mac@hotmail.com before the 1st day of the new month. Upon agreement, monthly payments will cease until I/we have fully recovered. Monthly payments will then resume, as will prior grades, skills and attendance.
Unauthorised tuition payment breaks will need to be resolved before training may continue.
CANCELLATION POLICY
I/we understand that to cancel membership with Fusion Martial Arts Centre Ltd, written notice must be submitted via email to fusion_mac@hotmail.com. Fusion Martial Arts Centre Ltd reserves the right to charge for tuition if no written notice has been given. Refunds may be considered on a case by case basis.
Tuition must be paid in lieu for the subsequent month if membership is cancelled within the 12 month period.
I/we understand that all records will be wiped after cancelling my/our membership and if training was to continue at a later date, progress would be lost.
MEDICAL CONDITIONS
I/we have no medical conditions to my/our knowledge that will impair my/our ability to train*.
*If you are unsure then you must first seek permission from your medical physician or GP before partaking in any form of sport or physical activity
FIRE EVACUATION AND EMERGENCY EXITS
I/we have been made aware of the Fusion Martial Arts Centre's fire evacuation procedure, the emergency exits have been highlighted and external roll call points explained.
ACCIDENT REPORTING
I understand that if I/we suffer an injury whilst training, I/we will report this immediately to a member of staff*. I/we will contact the Fusion Martial Arts Centre Ltd if medical attention is sought and required afterwards.
*Details of the incident shall be recorded and forwarded to H&S.
CHILD PROTECTION
I/we are aware that child saftey is an important value at the Fusion Martial Arts Centre Ltd and any concerns of neglect towards a childs mental or physical well-being will be reported to the relevant local agencies.
I/we are aware that spectating is only permitted by parents with children partaking in the class. *Our instructors are DBS checked and your child can be left after the session commences.
Please note: We cannot accept liability for children left unattended outside of our unit so please escort them to our reception and collect once the session has finished.
PHOTOGRAPHY
I/we acknowledge that under no circumstance should filming or photography occur within The Fusion Martial Arts Centre without seeking prior permission. This must be sought from the class instructor and any parties involved. I/we consent that Fusion Martial Arts Centre Ltd may capture images or videos during sessions which may be displayed on the website or social media channels*
*Content will not be shared with outside companies/people.
CORRESPONDENCE
I/we acknowledge that Fusion MAC utilises WhatsApp as the main source of communication when contacting members on mass. Instances may include:
Class cancellations/amendments
Grading information
Competition updates
Other important issues regarding the Centre.
I/we will not hold the Fusion MAC Ltd accountable for any missed communication if I/we choose not to join the group.
